Pretty good, seeing as how the majority of the rape, murder, and pillaging that has been going on the past 60 years has been carried out by the united states military and its affiliates and that the united states is the only country to use nuclear or uranium laced weapons on another country (victims include not only Japan but also Korea, Vietnam, Iraq and Yugoslavia), while the North Koreans have literally never hurt a fly since the Korean War.

I wouldn't trust them one second. Will they attack the US with a nuke in the near future? no. Trusting them with nukes is a completely stupid idea. They are run by a guy who has all the power of his country. Power corrupts people and North Korea has already done stupid shit in the passed. Imagine what would happen if they messed up with a nuke.

If anything there no country should have nukes. There is no need for them, they will only kill 10's if not 100's of thousands of civilians.

Depends. It could sure as hell end a country, probably a region, a continent if shit really went down. If shit REALLY went down, yeah, large parts of the world would probably become uninhabitable. The whole earth wouldn't be a Mad Max hellscape, because frankly there are parts of the world that aren't worth the trouble it is to turn those two keys.

This. Maybe I'm biased because I just read The Road but if a full-scale nuclear war took place, that's what I think earth would look like afterwards. It wouldn't even matter if certain parts of the earth didn't get bombed because of all the death clouds and grossness from the places that did get hit would cover the whole planet, and I think a lot of (most) living things would be done for.
