For those that are interested, Bp published their report on the findings of the internal investigation of the Macondo Blowout.
It's fairly complex (Tasche, you will probably be the only other NS who understands this fully) but there is a lot of detail in here about what happened.

Yeah i've not had time to go through it all yet.... just quick scan - some real interesting Lessons Learnt potentially with the Rig Design also with the gas venting and dispersion models.
But from looking at it - it seems the Cement Job is a major factor (and of course the BOP failures.. but i haven't look in to those in much detail.... )
Also - it was the cement job that was blamed for the blow out in W Austrailia last year - and that was also done by Halliburton, so they are not going to come out of this well also.
