Fully custom footbeds would be a really good start if you're not into getting new boots fitted. Mine were about $110 CAD to have made.
You said you're having problems with crushed toes, the footbeds will help keep your foot in a neutral position, and prevent your foot from flattening on impact/whenever. When your foot does this it forces the toes into the front of your boots. I'm guessing it'll do a lot in helping your other foot pain, but the best way to tell is to just go to a bootfitter and let him tell you what's up.
