im no pro, but it looks to me like youre taking a nice slow backswing, but as soon as you get to the top youre immediately firing the cannons back down. let your club slow down at the top of the backswing, pause at the top for half a second, and then accelerate all the way to the end of your follow through. if you made a chart of you club speed, it should look almost exactly the same as if you were throwing a ball in the air and letting it drop down again. for me, almost anytime my swing starts to get sloppy is because i start rushing it, and it looks like you might be doing the same thing. just remember you dont have to hit the ball hard at all, you just have to make good contact.
